It would be great and not a bad Idea, but there might be some issues that need to be considered:
incompatibility, not like organ incompatibility that we consider with general organ donation, as having genitals from the opposite model (even from a perfect donor)will have tissues that your body has never seen and will likely trigger immune responses and will necessitates stronger immunosuppression than a normal organ donation. that will put the patient at much higher risk of diseases.
Nerve connection,. Currently [genital]plasty doesn't chop off the parts just reshapes the tissues harming the nerves as little as possible so they still work and feel stuff, a transplant will cut the nerves and reattaching them is extremely difficult, and never perfect, this will likely result in genitals with little to no sensitivity.
3)scar tissue doesn't stretch well, so pregnancy with a transplanted set. uterus transplant has been successfully done and can carry pregnancies though c section, but were talking about all the set, which might make it really hard to potentially impossible for the pregnancy to grow and expand. (might, I'm talking out of my ass in this point)

I dunno, I know some folks that would rather size down. But either way, the consequences of passing out at a frat party would be much more compelling to the average fratbro at that point. You'd have collections of dong hanging from ceiling fans.

I get what you're saying, but I think in a world where we can swap genitals for 24hr, current medical concerns of our world don't factor in. :)
I did not say that as a downer, just bringing up issues, which paradoxically, if this operation began (as soon as we get over the tissue incompatibility, which might not be a real issue just something that I think might be) both other issues will improve as more effort and investment is put in those operation.
Would sort of suck for the people who want to do it the first few years (I am sure there are people who despite lack of sensitivity would prefer their desired genitals), But it will improve to the point that it might become reasonably mainstream.
Yeah, unfortunately it's not even realistic to do allografts with close relatives that are good tissue matches when it comes to something that needs the degree of nerve connection genitals do.
You might, hypothetically, figure out a way to graft a penis onto a clitoris in a functional way, though damned if I can imagine it being a better alternative than existing options. But it won't be a fully sensate organ . Even grafting penis to penis (which has been manged to an extent) nerves aren't 1:1 if you could make them align well and magically grow together.
And trying to graft in a full vagina and have it be more than decorative is even more daunting, imo. A vagina just isn't something you can dissect out and directly implant. The entire vascular network just isn't compatible, from what I've seen. Ignoring nerve connections just not being there, if you can't supply blood, you lose a majority of what makes a vagina work in the full sense of things. That's ignoring necrosis without a good blood supply.
Tbh though, my comment was more about the concept of human genitals actually being directly compatible in that way. As in, you snip one bit, move it over, and it clicks into place like legos with some extra glue. More of a joke than a reflection of transplant technology.
But holy shit, I would absolutely leave my genitals to some guy that needs/wants it. Like, if it was possible, please give my frank and beans to a man looking to transition! I won't be using them any more once I kick off. My eyes suck, and it's dubious what kind of shape my internal organs will be in by the time I die, but anyone is welcome to those, so why not my junk? Hell, give the block and tackle to a guy with micropenis, or that lost theirs for some reason. I'm an organ donor anyway, and if I can't be buried under a tree in the wild somewhere, I want cremation, so it doesn't matter what's left when it happens, use me up, dammit!
maybe not full genitals but uterus/testicles might actually work and be easier, they do not need nerve attachments and might act as permanent HRT. wont be functional for reproduction or sex, but might make the patient no longer need to take pills injections.
But unless we include nerve attachments for the tests trans men will never know what it feels to get kicked in the balls, which is probably for the best
