Why need 16 modifications, what are the different applications and how are they so different to each other that you need 16 modifications for a rocket?

Wouldnt (random number) 3 suffice?

It's more like 16 permutations of 3 modifications:

  • Number of strap-on solid rocket boosters (0, 2, 4, or 6)
  • Short vs Long payload fairing
  • LEO vs HEO optimized Centaur V

These can be mixed and matched based on payload mass, size, and target orbit.
