She's his mom he was with her. In the begining of the chp, when he saw the buttler he got mad. In the above frame if u take a close look to his hand (it's not abvious) he has a knife. The reason why he's sitting there is because he was battling with himself whether he should go kill him or not.
Funny thing is kojima did hallucinate a walking sim game and it was actually a fun game
