tonight radio-canada (the french-speaking counterpart to the CBC) was interviewing a lot of people from Iran, and one of the questions the interviewer asked was "so, with Khamenei dead, do you think people will go to the streets to protest their government and bring about regime change?" to which the interviewee just answered "no people are hiding and fleeing, Tehran is being bombed!"

As well as being too large and sick to do anything about what the government is doing, the photo is supposed to convey that they also look sad (teary eyes) and hopeless (distant gaze despite being close to camera and faint smile that isn't really a smile).
