All throughout video game history, some of the best details were included without any plan to, without permission, or even against the wishes of higher-ups.

For sure! For example, Gabe specifically asked for cake in Portal. He said "After each level we should reward players with some cake. Cake is the truth of our game." But the developers couldn't manage to render a good looking cake, so they lied to Gabe and shipped the game without. As punishment to the developers, Gabe killed Half Life 3.

Another example I can think of is the iconic dancing in the clubs in VtM: Bloodlines and the animation for it. Snuck in after hours after a spontaneous idea if I recall right.

The questionable consent in The Lusty Argonian Maid is totally intentional, but should be taken in context, because in lore it's

spoilera wealthy and influential NPC who, after spending lots of time doing a series of quests for him to advance in his faction, he hits you with a quest to take off all your equipment so he can see the player character naked, and if you refuse to do this you can't advance further in that quest line. This happens regardless of your character's gender. He's depicted as a sex pest and a creep in a way that feels very personal and elicits a strong negative reaction, because you can't say no without making a meaningful sacrifice within the game.
So if you play the game you're going to be inclined to view the author's intent with a strong negative bias.

I admit I did not pay attention to the lore and read as a fune porn book in-universe to not put too much thinking into.

I suppose this would make me the normie who doesn't realize the heinous stuff going on behind the funny joke, very on point.

I appreciate you providing this info, I just looked up this guy because I do not remember encountering him. He is originally from Morrowind, it turns out, and I did not play Morrowind yet so I knew none of the context. Thanks for taking what might have been intended to be an outrage bait those-damn-SJWs comment (or maybe it was intended innocently! I've been wrong before) and using it to jump off into something people could start an actual discussion about.
