"Conversations I imagine in my head that never actually happen to anyone"
It's happened to me. Several times.
Every time my wife has some medical thing where the symptoms or treatments could be effected by pregnancy. Although usually they just skip the sexually active question and go straight to "is there any possibility you could be pregnant?"
I've been present for a number of these during emergency rooms visits, and a few other situations where I had to drive her due to the use of anesthesia.
I'm a trans woman who has absolutely had this conversation
This happens literally every day in clinics.
