You're forced to create a user on lemmynsfw in order to see content, that's why they have so many users. pornlemmy.com is different, they don't put anything behind a firewall.

lemmy.world doesn't seem to have many established NSFW communities; they appear to be in separate instances. Is there any benefit to this or anything stopping lemmy.world from having NSFW communities? by skierra in c/nostupidquestions@lemmy.world

It's better to separate nsfw into their own instances, because then admins who don't want porn on their Lemmy servers can just block the offending instance.
