The gold standard in my experience is Sweden. They probably do the same thing in other countries, but that's the place I have visited.

Instead of having a single big open men's room with 2-3 toilets and a couple sinks, next to a single big open ladies' room with a similar layout, there is a short hallway with four doors on each side. Behind each door is a small restroom with its own toilet and sink. I think they each had a urinal too.

It is superior in every single way except maybe for initial construction cost. It has a higher number of toilets which are all gender neutral, on top of actual privacy and there not being a "bad time" to go if it's busy.

It makes total sense if you give a shit about people and aren't trying to penny pinch construction costs while making the facilities user-hostile so that peons don't spend too much time in there.
