It's a little weird after so many years to see the Doctor specifically called "Doctor Who" as a name for multiple sentences.
BILL: Sorry, what do you mean, it's his real name? Nobody knows the Doctor's real name.
MISSY: I do, because I grew up with him, and his real name is Doctor Who.

I thought so too. At least they realised that nobody in the younger segment would get the joke if they called him "Doctor OHO".
The book must be about Peter Cushing.
