Yes, it's a great game, and you start the game by being penitrated (by a bullet to the head)

Kinky, does that game require mods? Or should I just jump in
I'd recommend mods unless you also like being pegged by bugs and crashes. Also possibly something to help with combat, it's a little jank when not using V.A.T.S.

I recommend sections 1, 2, 3 for a first play through since they are about performance and stability
