One of my patients almost died from acute onset pancreatitis after eating 4 pounds of fudge in one sitting, so this could be a trick question.

I believe he has some substance abuse issues with alcohol. A lot of alcoholics can get some wires crossed when it comes to sugary foods.

It's typically Alcohol specific, and usually when people are trying to quit. Alcohol and sugar utilize a similar dopamine pathway and alcohol metabolizes into sugar so it can develop into a parallel addiction and metabolic dependency.
Lots of alcohol plus 4lbs of sugar, ouch.

In the long run I'm sure, but most kidney issues are a secondary symptom of cardio vascular disease and diabetes, really anything that elevates your blood pressure a bunch.
Are we talking about American candy bars or European candy bars? .... because if it's in Texas, the child is going to have instant diabetes and will probably lose both their feet ... if it's in Switzerland, they'll probably break their fingers trying to snap that many Toblerone bars

I got to work with that guy on set of a show. He was super cool and down to earth. Wouldn't stop preaching about diabetes but he was cool.
